C.A.R.E:
The difficulties experienced by older adults are too often accepted as a normal part of aging. Serious decline in the later years can often be attributed to a variety of factors including physical and mental disorders.
C.A.R.E (Coordinated Assessments & Resources for the Elderly) is designed to provide assistance to older adults whose independence is threatened by physical and mental difficulties.
Professionals from Elderly United are working at one downtown location to provide a comprehensive range of services to help older persons maintain the highest level of functioning, independence and quality of life possible.  Mental Health Services provides diagnostic assessment, med/somatic, and individual counseling at C.A.R.E. on a contractual basis.
C.A.R.E services include:

  • Gatekeeper - to train community workers to identify and refer at-risk individuals
  • Assessments - to determine problems, needs, strengths and limitations
  • Outreach - to access community resources
  • Counseling - to assist with problem solving and facilitate growth.
  • Guardianship - recruiting, training, and matching volunteers to serve as guardians.

Most services are provided at no charge, but no one is denied service because of their inability to pay. Many people are covered by their insurance plans, including Medicare, for diagnostic assessment and counseling services. For more information contact (937) 324-9000.

Homemaker / Personal Care:
Light housekeeping, laundry, assistance with personal hygiene, and essential errands are provided on a limited basis to prioritized clients. Client donations are requested to supplement the cost of the service. For more information, call (937) 324-9000.

MON AMI:
Mon Ami is a supportive program for persons age 60 and older who are isolated and alone. Persons who have limited opportunities to interact with others because of physical, mental or social isolation are encouraged to participate in the program.
Mon Ami provides:

  • Daily Outings
  • Support Groups
  • Socialization Activities
  • Volunteer Services

Volunteers are an integral part of Mon Ami serving in:

  • Respite Care - providing sitter/companion services to families with 24-hour care giving responsibility for an older family member.
  • Companion - providing support and friendship to older adults who have some difficulty in coping with getting older
  • Money Management - acting as representative payees and bill payers for persons who cannot manage their finances because of physical or mental impairments.
  • Friendly Visitation - becoming friends and companions to older persons who are lonely and have little outside contact or support.
  • Assisted Socializations- assisting staff with group socialization activities.
  • Support Groups - serving as peer counselors and group leaders.
  • Telephone Reassurance - making daily phone calls to isolated persons to insure their safety and provide socialization.
  • Guardianship - serving as a court appointed volunteer guardian to Medicaid recipients over age 60, who have no family able or willing.
